来自重症监护室的15例高毒力肺炎克雷伯菌肝脓肿病例分析

摘要


目的：分析高毒力肺炎克雷伯菌肝脓肿的临床特征，为临床诊疗提供经验。方法：回顾性研究15例肝脓肿患者的临床资料，包括临床表现、实验室检查、病原学检查、影像学检查和治疗转归等。结果：15例患者均为高毒力肺炎克雷伯菌感染致肝脓肿合并感染性休克，常见临床表现为发热、乏力、纳差、血小板减少、低蛋白血症、胸腔积液、白细胞增高、降钙素原增高等，多数患者合并糖尿病史，经过积极抗感染治疗和肝脓肿穿刺引流治疗、机械通气、控制血糖等对症处理。结论：目前肝脓肿主要致病菌为高毒力肺炎克雷伯杆菌，侵袭性强、病情进展迅速，抗感染治疗联合肝脓肿穿刺可有效改善预后
