骨科住院医师规范化培训的发展及影响因素分析

背景与目的:骨科住院医师规范化培训是培养合格骨科医师的关键环节,但对其能力发展轨迹的系统性研究尚显不足。本研究旨在通过360度评估体系,探讨骨科住院医师在三年培训周期内的能力变化规律及其影响因素。方法:采用回顾性队列研究设计,收集2019年7月至2024年9月期间北京大学人民医院骨科住院医师的360度评估数据。评估内容包括带教老师对住院医师评价、住院医师对带教老师评价以及护士对住院医师评价。采用配对t检验分析纵向变化,通过多元线性回归探讨影响因素。结果:共纳入59名住院医师和20名带教老师,获得364份有效评估问卷。住院医师总体评分在前两年显著上升(第一年:82.67±8.26分,第二年:89.85±6.03分,P<0.001),第三年趋于稳定(91.77±4.03分,P>0.05)。临床技能提升幅度明显高于非临床技能。学历是早期评分的主要影响因素(P<0.05),而性别、年龄等因素在后期影响有限。结论:骨科住院医师能力发展呈现“快速提升-平台稳定”的双阶段模式,临床技能改善优于非临床胜任力。培训项目应针对不同阶段特点优化教学策略,特别关注高年资住院医师的持续发展和非临床技能培养。
